    Current Clients: If you need to change your appointment time due to things like work schedule, sickness, etc. please do so 24 hours in advance. Do not use this form to request appointment changes at the last minute. If you are not on time for your appointment, you could be violated. If you have a last-minute issue such as car trouble, call Kay at 910-859-1150 or Maggie at 910-640-2318 so that we can help you find a solution.

    IF YOU MAY HAVE BEEN EXPOSED TO COVID-19 OR YOU HAVE SYMPTOMS SUCH AS A HIGH FEVER, COUGH, AND/OR LOSS OF TASTE OR SMELL, PLEASE DO NOT COME TO THE OFFICE. Call Kay (910-859-1150) to discuss solutions. If your monitoring is court-ordered, a Covid test may be required by the courts to document your missed appointment.
